PATIENTS ARE SEEN BY APPOINTMENT ONLY

Appointment schedules vary according to the provider of service.  We are happy to reserve a time in your provider’s schedule, however, in consideration for others, we do request at least a 24-hour notice prior to cancellation of any appointment.  We do understand that there are circumstances that may prevent you from keeping your appointment.  If you provide us with as much notice as possible, we may be able to contact another patient who needs an appointment.  Appointments fill quickly, and cancelling with less than 24-hours notice does not allow enough time to schedule another patient in need of treatment.  Therefore, a cancellation or no-show fee will apply if our office is not notified that you will be unable to make your appointment.

ALL NEW PATIENT APPOINTMENTS MUST BE SCHEDULED BY PHONE.

Follow up appointments with Dr. Gripon or Beth Carson may be scheduled by phone or online.

All appointments with Ben Bell, LPC & Jennifer Esclovon, LPC must be scheduled by phone.

In an effort to better serve our patients, we have implemented an automated voice attendant to assist with your telephone requests.  We make every effort to answer your telephone call promptly, however, when our staff is busy assisting patients, your call will be directed to the automated attendant, at which time you will select the option that best meets your need:

BILLING AND FEES

Your patient FINANCIAL RESPONSIBILITY is due at the time of service.

For your convenience we accept cash, checks and credit cards.

FOLLOW UP VISITS
All follow up visits will be scheduled at least every 3 months.
Many patients will be required to be seen every 2 months.

We will no longer refill controlled medications by fax:
You will be given prescriptions with refills to last until you next visit.

If you miss your appointment, medication will not be refilled until next scheduled appointment.

There will be no exceptions.

If you are prescribed medication from our office:
You may be subject to random drug screens. Failure to comply will result in termination of care.

NO SHOW VISITS: 
Failure to keep an appointment will result in charge of full office visit,

unless cancellation is received one day in advance.
3 no show visits will result in termination of care from this office.
